Stems thin and wiry, erect or straggly, 200–600 × 3 mm, often yellowish, leafy, leaf sheaths with short hairs. Leaves narrowly lanceolate, 40–60 × 6 mm, stiff, dark green to yellowish, unequally notched. Flowers not opening widely (sometimes remaining closed), 1–2 in leaf axils, 5–10 × 3–5 mm, white, cream, pale green or greenish yellow, sometimes with orange veins, labellum white. Dorsal sepal 4–5 × 3 mm. Lateral sepals 4–5 × 3 mm, tips incurved. Petals 4–5 × 1 mm. Labellum c. 13 × 5 mm; lateral lobes narrow, erect; midlobe ovate, with densely packed dark warts, notched, with 3 ridges, outer ones densely warty.
Widespread, but disjunct and localised; found growing in wet sites among sedges and rushes in sandy and peaty soils in swamps, springs, seepage areas and beside streams, even in the beds of sluggish streams.
