Leaves 4–8, 70–120 × 1–3 mm, straight. Flower stem 120–200 mm, 1–3-flowered. Flowers suberect to porrect, 15–25 mm across, pale yellow, labellum lightly stained with tan-brown markings and streaks, few streaks on dorsal sepal. Dorsal sepal obliquely erect, 9–15 × 4–8 mm. Lateral sepals obliquely deflexed below labellum, 13–30 × 1.5–3 mm. Petals incurved to spreading; stalk 3–5 mm long, green to brown; blade ovate to elliptic, 7–15 × 3.5–7 mm. Labellum 10–18 mm long; lateral lobes 2–3.5 × 1–1.5 mm, minutely papillate, toothed; midlobe narrowly ovate, 7–14 × 6–10 mm with prominent neck. Callus thick, ridges 2, broad, 3–5 mm long, densely papillate.
Known from few sites; at some sites it grows among grasses and forbs on windswept grassland in red-brown clay loam; at Mt Jephcott it grows on top of a hill among Cambrian rock and sheoaks; at Mt Kerang it grows on slopes in yellow box woodland. Much of its habitat is remnant grassland that is slowly recovering from clearing and stock grazing.
