Plants 400–600 mm tall. Free part of leaf to 150–450 × 4–6 mm. Spike 80–150 mm long, 10–40-flowered. Ovary sessile, green, shiny. Flowers crowded, scented, 10–13 × 9–11 mm, greenish with light brownish or reddish markings, petals with pale margins, labellum white or pinkish, callus green to yellowish green. Dorsal sepal 7–8 × 3–4 mm, porrect or incurved. Lateral sepals free, 7–9 × 2 mm, widely divergent. Petals 6–9 × 1–1.4 mm, incurved or spreading. Labellum shortly stalked, ovate-oblong, 6–9 × 5–6 mm, recurved near middle, constricted above middle, margins strongly wavy/crinkled. Callus broad, fleshy, channelled, extending nearly to labellum apex, blunt or notched.
Originally recorded as locally abundant at Gorae West near Portland from ‘Flat, heavily timbered country, rather rough, in black loam soil.’ The species is now highly localised and known from only 2 sites; near Condah it grows among rushes and tussocks in and around the margins of a swamp in water, whereas near Dunkeld it grows in wet grassland and grassy woodland; found growing in seasonally wet, heavy, dark grey to black loam.
