Leaves erect, 6–15 × 5–8 mm, pale green to yellowish, thin-textured, margins crinkled, on petioles 30–50 mm long; in flowering plants leaf attached to flower stem well above soil level. Flower stem 50–150 mm long, fleshy, brittle, 1–4-flowered. Flowers horizontal or nodding, 5–7 × 6–8 mm, green with red markings. Dorsal sepal 6.5–8 × 4 mm, hooded over column. Lateral sepals 6.5–8 × 2 mm, projecting forwards or decurved. Petals c. 2.5 × 1 mm, spreading or decurved. Labellum broadly heart-shaped, 5–5.5 × 5–6 mm, blunt, very smooth, folded along mid-line, minutely papillate near apex, tip deflexed. Callus of 2 obscure central plates.
Relatively widespread and common in the west of Tas., but generally localised and habitat-specific. It grows in permanently wet forests, especially myrtle beech forest and rainforest; less commonly in tall, wet sclerophyll forest. The plants form small colonies in substrates that are continually moist, including pads of thick moss and liverworts on the bases of trees and rocks, on rotting logs and in accumulations of decaying beech leaves.
