Leaves 40–60 × 15–20 mm. Flower stem 60–100 mm long. Flower stiffly erect, 20–24 × 20–25 mm, green to purplish brown with main labellum calli shiny black. Dorsal sepal spathulate, 20–24 × 7–11 mm; apical club 1–1.5 mm long. Lateral sepals projecting forwards below labellum, 15–20 × 2 mm; apical clubs 1–1.5 mm long. Petals incurved near column, 15–17 × 4.5–6 mm, strongly curved. Labellum held nearly horizontal, broadly ovate to heart-shaped, 10–13 × 10–15 mm, margins with pale green band. Calli in narrow central line on slightly raised plate, column-like; main glands with large, swollen, shiny black heads. Column 15–18 × 5–6 mm.
Relatively widespread, but uncommon and mainly found in coastal and near-coastal areas. Grows among shrubs, tussocks and litter in forest and coastal scrub in freely draining, sandy loam; also among ferns and in deep forest litter in tall, moist to wet forest, including rainforest dominated by myrtle beech, in well-structured clay loam.
