Leaves 3–5, scattered up stem, 30–60 x 15–30mm, rich green, fleshy, margins flat or crinkled. Flower stem 120–40 tall, scabrous. Flower erect, 25–30 x 9–12mm, translucent white with bold dark green lines and markings. Dorsal sepal of similar length to petals, tapered to short point. Lateral sepals loosely embracing galea and separated from petals by wide side gap, abruptly bent backwards, sinus bulging prominently in side view, broadly notched in front view. Free points 13–1 long, swept back and extending well past hood. Petals shortly pointed. Labellum curved near apex, protruding prominently from sinus when set, 12–14 x 3–3.5mm, widest near base then tapered, dark green to brown, blunt.
Locally frequent in foothills and ranges, extending to some montane areas but not alpine regions; commonly seen growing on sheltered slopes, gullies and stream banks in moist humid forests, especially tall forests with grassy understorey, wet sclerophyll forest and fern gullies in rich loam, occasionally in sphagnum bogs; less commonly found on moist flats in lowlands, growing in heavy soil and under coastal scrub and teatree thickets on S slopes and swales of stabilised dunes in sand.
