Leaves 3–5, in loose rosette or scattered up stem, 30–80 x 15–25mm, dark green, fleshy, margins entire or wavy. Flower stem 200–40 tall, stout, scabrous. Flower erect, 40–50 x 17–22mm, translucent white with dark green stripes and suffusions. Dorsal sepal of similar length to petals, shortly pointed. Lateral sepals loosely embracing galea and separated from petals by narrow side gap. Sinus scabrous, slightly bulging in side view, broadly and deeply notched in front view. Free points 15–2 long, held high above hood. Petals pointed. Labellum sharply curved near apex, protruding prominently from sinus when set, 16–20 x 3.5–4.5mm, green with brown margins, apex brown, blunt.
Locally abundant in moist to wet montane, subalpine and alpine areas, growing among grass and sedges on sheltered ridges, S slopes, near streams, grassy flats and swamp margins in snowgum woodland and tall forests in rich moisture-retentive loams; occasional plants grow in shallow running water and mud; also in sphagnum mounds.
