Rosette leaves 4–5, elliptic, 6–60 x 8–20mm, dark green, margins strongly crinkled. Flower stem 200–35 tall, smooth. Flower erect, 40–50 x 18–25mm, translucent white with green and red-brown markings. Dorsal sepal slightly longer than petals, sharply pointed. Lateral sepals loosely embracing galea and separated from petals by wide side gap. Sinus bulging in side view, nearly flat in front view with shallow central notch. Free points 20–2 long, swept back, tips sometimes curved forwards. Petals flared, tips pointed. Labellum nearly straight, not protruding from sinus when set, 15–18 x 4–5mm, widest just above middle then tapered, brownish.
Localised in ranges and tablelands, growing among grass, shrubs and bracken on sheltered slopes and gullies in tall open forest in freely draining gravelly loam; on W slopes it grows in or close to small stream beds and wet sites.
